Helpful Tips

Most browsers have a downloads list. Look for a download icon, usually near the top-right corner of the browser window.

Download the form again

If you cannot find the file, return to the Forms & Downloads page and download the form again. Choose a location that is easy to find, such as Downloads, Desktop, Files, My Files, or a folder named BTH Forms.
