Monthly FTW Forms

These three monthly forms are the most commonly used for First-to-Work participation:

  • FTW Class Attendance
  • FTW Unsupervised Study Timesheet
  • FTW Monitored Study Sessions

Download these forms from the Monthly FTW Forms section.

Important: Save Forms First

Some browsers let you type directly into a PDF preview, but your answers may not save correctly.

For best results, download the form before filling it out. Open the saved file, complete it, save a finished copy, and reopen it to make sure your answers are visible.

For monthly forms, keep one blank template and one completed final copy each month.

Helpful Tips

Avoid Paid Subscription Prompts

You should not need to purchase a paid PDF editor, Adobe Acrobat Pro subscription, DocuSign account, or other e-signature subscription just to complete and return a FTW form.

  • Do not click Start free trial, Upgrade, or Subscribe unless you personally choose to.
  • Close pop-ups asking you to pay for editing or signing tools.
  • Do not upload private FTW forms to third-party signing websites unless BTH staff instructs you to.
