Quick Steps
Download the PDF
Click the form link and save the file to your computer, phone, or tablet.
Open Downloads
Look for the file in Downloads, Files, Finder, File Explorer, or My Files.
Save a Draft Copy
For monthly forms, save one blank draft/template copy before filling it out.
Save the Final Copy
After completing the form, save a final copy with your name before emailing it.

Use Finder and open the Downloads folder to find your saved PDF.

Use this guide if you are downloading forms on an Apple iPhone, iPad, Android phone, or Android tablet.

Helpful Tips
- If you just downloaded the form, sort your Downloads folder by newest file first.
- If you are using a Windows PC, you can press Ctrl + J in your browser to view recent downloads.
- If you are using a Mac, you can also check the Downloads stack near the Trash icon in the Dock.
- If you are using a phone, look in Files, Downloads, or My Files.
- After you find the PDF, save a copy with your name before emailing it.
If You Still Cannot Find the File
Check your browser downloads
Most browsers have a downloads list. Look for a download icon, usually near the top-right corner of the browser window.
Search your computer or phone
Search for part of the file name, or search for .pdf to show PDF files saved on your device.
Download the form again
If you cannot find the file, return to the Forms & Downloads page and download the form again. Choose a location that is easy to find, such as Downloads, Desktop, Files, My Files, or a folder named BTH Forms.
