Appointment Preparation

To help your appointment go smoothly, please complete the New Participant Form and gather the items listed below before meeting with your Bridge to Hope (BTH) campus contact.

What to Have Ready

First to Work (FTW) Case Manager Contact Information

Have your case manager鈥檚 name, email, phone number, and unit ready.

Processing Center Information

This is the office where you applied for financial assistance and SNAP.

Class Schedule

Bring or have access to your current class schedule.

Work Availability

Bring the hours you are available to work if you are seeking campus employment.

Forms Your Campus Contact May Review

Consent to Release Information to DHS

Allows information to be released to DHS-FTW when needed for BTH participation.

Consent to Review/Release Information from DHS-FTW to BTH

Allows DHS FTW information to be reviewed or released to BTH.

DHS Form 1465

Student Employment Documents

If you are placed in campus employment, you will eventually need a photo ID and your actual Social Security card for the I-9 form.
